Nursing Services
RIVEON OFFERS CLIENTS ACCESS TO NURSES WHO CAN ASSIST WITH CERTAIN MEDICATION-RELATED NEEDS.
REASONS TO USE NURSING SERVICES:
- Review and discuss medications and medical concerns
- Review hospital discharge information
- Discuss possible medication side effects
- Receive a missed injection
- Complete needed items for continued treatment
(Individualized Service Plans, Nursing/Health Assessments)
YEARLY REQUIREMENTS
Please bring your insurance information and your identification to these visits.
- Nursing Health Assessment—to be completed once a year, onsite, by a nurse.
- Individualized Service Plan (ISP) to be completed every six months and updated as needed.
These yearly requirements can be taken care of on the same day you see your provider. Ask your nurse for more details.
GUIDELINES FOR VISITS
Please arrive 10 minutes prior to scheduled appointments. If you need to cancel, reschedule or miss an appointment:
- You must do so at least 24 hours in advance of the scheduled appointment by calling: 440.204.4224
- If you do not call to cancel within at least 24 hours or if you simply do not come, we will note that you “no showed” for your appointment.
If you miss your provider’s phone call for your telehealth appointment:
within 10 minutes of your initial appointment time, inform the front desk that you missed a call and request they help you to connect.
You are responsible for scheduling all follow-up appointments.
If you miss an appointment, you must have another appointment scheduled in order for us to discuss your medications with you.
